Personal Care Services

Primary Home Care (PHC) – This program is a non-technical, medically related personal care service provided to adults whose health problems cause them to be functionally limited in performing activities of daily living, according to a statement of medical need.

Family Care (FC) – This program is a non-skilled, non-technical attendant care service for eligible adults who are functionally limited in performing activities of daily living. Primary Home Care provider agencies have the option of providing these services. Family Care services are provided by a Personal Care Attendant and do not require the supervision of a registered nurse.

Community Based Alternatives (CBA) – This program provides home and community-based services to aged and disabled adults as cost-effective alternatives to institutional care in nursing facilities (Personal Assistance Services only).

Community Assistance Services (CAS) – This program is a non-technical, non-skilled service providing in-home attendant services to individuals with an approved medical need for assistance with personal care tasks. CAS services are available to eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living according to a practitioner’s statement of medical need. CAS services are provided by a Personal Care Attendant.
